TERAPIA POR PRESSÃO NEGATIVA COMO UMA ABORDAGEM EFICAZ PARA O MANEJO DE FÍSTULAS GASTROINTESTINAIS PÓS-GASTRECTOMIA

Resumo


Introdução: A terapia por pressão negativa já tem sido utilizada na prática médica há 25 anos, todavia ainda é uma terapia inexplorada, com grande potencial para se tornar um tratamento de complicações pós-operatórias. Objetivo: Analisar a correção de uma fístula gastrointestinal, resultante de uma complicação pós-operatória, por meio da terapia de pressão negativa. Métodos: Esta pesquisa seguirá o CARE Checklist, que consiste em 13 itens. Visando garantir o maior nível de evidência, uma busca da literatura foi realizada por meio da base de dados Medical Literature Analysis and Retrieval System Online (MEDLINE) via PubMed. Foram selecionados 4 artigos cujo foco foi o uso da terapia à vácuo em complicações pós-operatórias no TGI. Resultados: Foi exposto o caso do paciente R. L com diagnóstico de câncer gástrico, que após gastrectomia subtotal, teve como complicação uma fístula gastrointestinal. A fim de tratamento, uma sonda naso-enteral, especialmente preparada com orifícios, foi introduzida no lúmen gástrico residual, acoplada à uma bomba de vácuo, que produziu pressão negativa continuamente durante 10 dias. Após esse período, foi realizada uma endoscopia digestiva alta que trouxe como resultado o fechamento da fístula, confirmando o sucesso do tratamento. Com a finalidade de garantir maior nível de evidência, 4 artigos que analisaram técnicas semelhantes foram descritos. Conclusões: A terapia à vácuo é um procedimento minimamente invasivo com grande potencial em tratamentos, além de demonstrar ser um procedimento seguro, de baixo custo e que confere maior qualidade de vida ao paciente, diminuindo tempo de internação e melhorando as condições pós-operatórias.

FROM RISK TO RECOVERY: SUCCESSFUL MANAGEMENT OF A GASTROINTESTINAL FISTULA WITH NEGATIVE PRESSURE THERAPY AFTER GASTRECTOMY

Background: Negative pressure therapy has been used in medical practice for 25 years, but it is still unexplored and has great potential becoming a treatment for postoperative complications. Objective: Analyze the resolution of gastrointestinal fistula postoperative complications of gastric cancer through negative pressure therapy. Methods: This research will follow the CARE Checklist, which consists of 13 items that aims to help researchers write effective case reports. Furthermore, to obtain a higher level of evidence, research was conducted using the Medical Literature Analysis and Retrieval System Online (MEDLINE) via PubMed from which four articles that focused on the use of vacuum therapy for postoperative complications were selected. Results: The case of patient R.L., a 60-year-old diagnosed with gastric cancer, was presented. After undergoing a subtotal gastrectomy for oncological resolution, the patient developed a complication consisting of a gastrointestinal fistula. To treat this complication, a specially prepared nasoenteral tube with openings was introduced into the residual gastric lumen and connected to a negative pressure machine, producing continuous negative pressure for 10 days. After this period, an upper digestive endoscopy (UDE) was performed, which resulted in the closure of the fistula following the use of vacuum therapy, confirming the success of the treatment. Conclusions: Vacuum therapy is a minimally invasive procedure with potential for treating postoperative complications. In addition, it has proven to be a safe, low-cost procedure that enhances quality of life for patients by reducing hospitalization time and improving postoperative conditions.
